The duty log.
The Duty Log is a note presumably written by one of the staff members of Riverside Motel in Silent Hill: Origins. It refers to the discovery of Richard Grady's corpse in Room 500. According to the note, Travis Grady had been in the room with the body for ten hours, unable to accept the fact that his father had died.
It can be found in the first floor manager's office of the motel.

Door was open. Inside, sweet
Jesus! Stench made me puke. The
kid, man. The kid! Just stood
there. They said he'd been there
for ten hours! I ended up crying
my eyes out. Shit like this
shouldn't happen. The police
have got the room locked up for
now, I'm going home."
